SERVICES

Home care packages can help you access services at home which can improve your wellbeing and help you stay independent. You may be eligible to receive home care services such as:

  • Personal services
  • Nutrition, hydration, meal preparation and diet
  • Continence management
  • Mobility and dexterity
  • Nursing, allied health and therapy services
  • Transport and personal assistance
  • Management of skin integrity

 

WHO PAYS FOR SERVICES?

The Australian Government subsidises many different types of Aged Care Services. You are expected to contribute to the cost if you can.

How much you pay may depend on: your financial situation; the number and types of services you receive; and the service provider.

The Program provides a subsidy towards a package of care, services, and case management to meet your personal needs.

WHO IS ELIGIBLE?

Each program has broad eligibility criteria and an assessment process to determine your needs and eligibility. This process helps to ensure everyone receives the care and support they need.

Eligibility requirements may include minimum aged and a decreased ability to perform basic daily living tasks without extra help.

Lakes Home Care services are available to residents who have aged related conditions and their Carers who reside within the Fleurieu Region, including Milang, Clayton, Point Sturt, Langhorne Creek, Finniss, Belvidere, Strathalbyn, Murray Mallee, Hills Mallee and other local areas.

Your eligibility for LAKES HOME CARE packages is determined by you calling My Aged Care on 1800 200 422 or making contact via www.myagedcare.gov.au

Our Coordinator will then receive your referral from My Aged Care and will arrange a Home Visit with you, at a mutually convenient time.  The Coordinator will explain what services are available and discuss your options.
