Care at home, from locals who know you

Support at Home is the Australian Government’s aged care program for older people who want to stay living independently in their own home. It replaced the Home Care Packages Program (and Short-Term Restorative Care) on 1 November 2025.

Lakes Home Care is a Support at Home provider. We are a not-for-profit run by caring locals for our community across Milang, Clayton, Strathalbyn and the wider Fleurieu region.

How Support at Home works

An aged care assessor will meet with you to work out what help you need. You will then be given one of the following:

  • Ongoing services, with a classification from Level 1 to Level 8 depending on your needs. Each level comes with a quarterly budget, so the funding follows your care as your needs change during the year.
  • Short-term support, such as the Restorative Care Pathway (up to 16 weeks of help to regain strength and independence, for example after a hospital stay) or the End-of-Life Pathway (extra support at home in your last months).
  • Assistive technology and home modifications, such as equipment, aids or changes to your home that help you stay safe and independent.

Once you’re approved, you choose your provider. We’ll then work with you to create a care plan and a service agreement built around your goals.

What services can you receive?

Support at Home services fall into three categories:

  • Clinical supports: nursing, allied health (such as physiotherapy, podiatry and occupational therapy), and, from 1 October 2026, personal care, which includes help with showering, dressing, grooming, toileting and continence.
  • Independence: support that helps you stay active and connected, such as transport, social support, community participation and respite.
  • Everyday living: help around the home, such as cleaning, laundry, shopping, meal preparation, gardening and home maintenance.

Care management is part of every Support at Home arrangement. Your Lakes Home Care coordinator will check in with you, review your plan, and make sure your services keep up with your needs.

What will it cost?

The government pays for most of the cost of your services. Depending on your income and assets, you may pay a contribution towards some of them:

  • Clinical supports are fully funded. You pay no contribution. From 1 October 2026 this includes personal care.
  • Independence and everyday living services may attract a contribution. The amount depends on your pension status and means assessment, set by Services Australia.
  • If you were already on a Home Care Package, on the waiting list, or assessed as eligible before 12 September 2024, you won’t pay more than you would have under the old arrangements.

You only pay contributions on the services you actually receive. There are no ongoing package or exit fees. You can see our current prices on our Fees page.

Moving across from a Home Care Package

If you had a Home Care Package with us, it has already moved across to Support at Home. Your services continue, and any unspent funds you had carried over remain available for your care. If you have questions about your budget or statement, please call us. We’re happy to go through it with you.

How to get started

  1. Contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to arrange an assessment.
  2. Once you’re approved, let My Aged Care know you would like Lakes Home Care as your provider, or give us a call.
  3. A coordinator will visit you at home to talk through your needs and set up your care plan.
